• Failure to maintain required equipment. Handwashing stations had no hot water. (Repeated deficiency)
• Unlicensed, unqualified, untrained staff providing patient care. (Repeated Deficiency)
• Failure to ensure staff are properly trained.
• Failure to adopt, follow, and or periodically review health and safety protocols.
• Failure to monitor patient vital signs. (Repeated deficiency)
• Failure to accurately document patient records. (Repeated Deficiency)
• Failure to document probable gestational age of the fetus.
• Failure to provide toll-free number to report complaints, neglect, or fraud. (Repeated Deficiency)
• Failure to document background screening on Administrator and Financial officer.
• Failure to document informed consent.
