• Expired medications and supplies. Crash cart was stocked with out of date medications. (Repeated deficiency)
• Failure to adopt, follow, and or periodically review health and safety protocols.
• Failure to ensure staff are properly trained.
• Failure to maintain required equipment. Mechanical suction machine, defibrillator, autoclave, and other emergency machines showed no evidence of ever being inspected or routinely maintained. (Repeated deficiency)
• Failure to train, comply with, and ensure infection control standards.
• Failure to ensure staff maintain proper license.
• Unlicensed, unqualified, untrained staff providing patient care.
