Health Violations

  • Physician failed to maintain a complete and accurate record: no history and physical, no signed consent for the abortion procedure by the doctor in 8 out of 10 charts reviewed
  • No policy for cleaning and sterilizing instruments.
  • The physician responsible for performing the abortion procedures on patients failed to sign a consent form for surgical abortion procedures. This form explains to patients the risks involved in an aboriton procedure including death, hemorrhage, infection, failed abortion attempt, organ injury to uterus or cervix, blood clots, and allergic reactions. This form also explains the procedure to the patient.
  • The facility failed to ensure that privacy was provided to patients in the recovery room. There were no privacy screens, curtains, or barriers between the recovery chairs.
  • Complete and accurate medical records were not maintained by staff. History and physical examination was not documented in 100% of the charts reviewed.
  • The facility had no policy requiring history and physical examination on patients receiving medication abortion.
  • The facility did not follow standard procedures for infection control. Staff failed to clean instruments to ensure safety and prevent the transmission of infections.
