Failure to comply with building standards. No hands free handwashing sink in facility.
Failure to adopt/follow/ and or periodically review health and safety protocols. Repeated deficiency.
Failure to ensure staff are properly trained.
Expired medications and supplies. Repeated deficiency.
Failure to ensure a safe and sanitary environment. Medical supplies were left open and damaged still in circulation to be used.
Unlicensed/unqualified/untrained staff providing patient care. After a 2nd trimester abortion a doctor is supposed to sign off on patient’s recovery, noting if they are stable and ready to be discharged. Clinic staff took vitals and discharged patients without a doctor being involved.
Clinic did not have any nurses on staff.
Failure to purchase and maintain required equipment.
Failure to document background screenings for employees.The Administrator and Financial Officer were not on the clearinghouse roster.
Failure to provide the required toll-free numbers to call for complaint, neglect, or fraud.
