• Expired Medications – The majority of emergency medications on the crash cart were expired. When questioned, the director stated that the medications on the crash cart were restocked every two years so no one ever checked them.
• A patient came into the clinic to have a medication abortion. She was given information on medication abortion and signed paperwork to have a medication abortion but a surgical abortion was performed without her consent.
