- Failure to purchase and maintain required equipment. Emergency equipment had not been calibrated in two years.
- Failure to adopt/follow/and/or periodically review health and safety protocols.
- Failure to properly train staff.
- Unlicensed, unqualified, untrained staff providing patient care. Including an expired license for one of the staff as well as several unlicensed staff performing skills that require a license.
