Health Violations

  • Staff was administering orally an injectable form of methotrexate for medication abortions, which results in malabsorption of the dose and a continuing pregnancy. The physician shunned responsibility by saying, “I don’t order. The clinic decides. I just sign.” He said he had been working at the clinic for 14 years and until 2 years ago, the staff was administering methotrexate intramuscularly. Oral or injectable is not indicated on the order, but he had never seen the methotrexate pill administered in the clinic.
  • Physician did not maintain accurate records to include operative notes.
  • Physician performing abortions failed to note the examination of the products of conception prior to discharge of the patient, leaving the potention for retained POC and risk of infection.
  • Staff failed to clean facility autoclave.
