• Failure to purchase and maintain required equipment.
  
• Failure to comply with building standards. There was one handwashing sink in the procedure room and it was missing a knob with only the central screw showing to turn the hot water on. This is the sink the physician uses.
  
• Failed to maintain an accurate record on patients. The physician marked that a patient was having a first trimester abortion when she actually had a second trimester abortion.
  
• Failure to keep updated policies and procedures documented and enforced.
